Adaptive platform trials are an innovative clinical trial design, where a Core (or 'Master') Protocol outlines the framework of the trial within which, all domains (similar to traditional clinical trials) are run.
This type of trial design allows accumulated data to be reviewed at pre-specified time-points (called 'interim analyses'). At each interim analysis, data is reviewed to assess the efficacy of treatments within each domain. A team of statisticians and clinicians who are independent of the trial, use pre-specified decision rules to determine whether the threshold for adaptations has been met or if sufficient data has been collected to answer the research question.
Adaptations to a domain might include the addition of new treatments, the removal of poorer-performing treatments, and/or changes to how participants are randomised to each domain.
The 'platform' of an adaptive platform trial can be likened to large, multi-use facilities, like Melbourne’s Cricket Ground or Auckland’s Eden Park. While expensive to build, once the structure, carpark and workforce are established, these facilities can be used for all sorts of events.
Adaptive platform trials have a single core protocol, single primary outcome, and coordinated infrastructure (including data management, statistical support, project management, health economics, etc.). This enables research questions can be added quickly and answered efficiently.
